    The Baller Academy Policy. No information will be shared.  Player Fees are usually sponsored by corporate friends and  associates.  • Every player must purchase and maintain their own uniform package at a cost of $325.00.  Elementary Packages cost 200.00 and includes a shooting shirt and reversible uniform. Used  uniform can be purchased if available. Limited numbers and sizes are available. If a player is  assigned a uniform, it must be returned after each game unless approved by a  coach of official or The Baller Academy. If a uniform or other item is not returned as promised,  The Baller Academy will pursue charges of theft of property unless the uniform or item is paid for in full. Player who don’t return items neglect other students from participating. • All player must have a legible report card, birth certificate and $16.00 AAU card prior to  participating in any of our events.  • An initial parent or guardian meeting is mandatory before any player can participate in a Baller  Academy event. • Mandatory Parents meeting are required to ensure that all the parents and guardians are  abreast to current events and concerns of THE BALLER ACADEMY. These meeting will be use to  voice any concerns as well as discuss Baller Academy business. Group text will be used on most  occasions. • Three unexcused absences and a player will be removed from our roster. Call your immediate coach or 713-557-0781 if you will be late or tardy. You may also text Coach Hardy or your  assigned head coach. • We will require students that are failing to attend our required tutorial session prior to practice as well as proof that you are attending school tutorial being offered. • We do not promise playing time or guaranteed minutes. We are a competitive amateur athletic organization  and playing time is earned. Coach’s decisions are final but may be discussed.  

   AMATEUR MINOR ATHLETIC WAIVER AND RELEASE OF LIABILITY In consideration of being allowed to participate in any way in THE BALLER ACADEMY athletics/sports program, and related events and activities, the parent or guardian: 1. Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate. 2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. 3. Assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent total disability or death. Child image may be used for advertising purposes.  4. Release, waive, discharge and covenant not to sue THE BALLER ACADEMY, its affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise. I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY. • All game, fundraising, travel, notices and other information will be posted on site. • Revised 11/07/2021
